The Milano is built on Birkenstock's signature contoured cork-latex footbed, which molds to the wearer's foot over time. The style is available in more than 45 material variants including natural leather, oiled leather, nubuck, suede, and synthetic options. Birkenstock offers the Milano in two footbed configurations: the Original Footbed with firm arch support and the Soft Footbed with additional foam cushioning.

The Milano's three-strap configuration consists of two straps positioned across the instep and forefoot, with a third strap wrapping around the ankle and heel. All three straps feature adjustable metal buckles that allow wearers to customize the fit. This closed-back design provides added stability compared to slip-on sandal styles, keeping the foot securely positioned during walking and other activities.

The ankle strap addition transforms the two-strap Arizona into the Milano, adding approximately one ounce to the overall weight. While the Milano weighs nine ounces compared to the Arizona's eight ounces, the closed-back design offers enhanced support for individuals with high arches or those requiring additional foot stability. The ankle strap also enables the Milano to be worn while driving, unlike fully open designs.

The Milano footbed features seven distinct layers designed to provide support and conform to individual foot shapes over time. From bottom to top, the construction includes an EVA sole, a strengthening jute layer, a cork and natural latex core, and a suede footbed liner. The footbed incorporates pronounced arch support, a deep heel cup, and a roomy toe box. A toe bar positioned near the front of the footbed encourages a natural gripping motion during wear. The cork layer naturally molds to foot contours, creating a personalized fit that develops with continued use.

Birkenstock offers the Milano in two footbed variants. The Original Footbed provides the standard seven-layer construction with firm arch support, requiring a break-in period as the cork adjusts to the wearer's foot shape. The Soft Footbed includes an additional layer of latex foam containing millions of tiny air bubbles, positioned between the suede liner and upper jute layer. This extra cushioning offers increased comfort immediately upon wear but provides slightly less arch support than the Original Footbed. The foam layer may also create a more snug fit in the toe box area.

The Milano collection spans more than 45 material variants across different upper options. Natural leather, oiled leather, and nubuck leather represent the traditional material choices, while Birko-Flor and Birkibuc provide synthetic alternatives. Premium editions include suede with shearling lining, while special releases feature patent leather and embossed finishes. Additional upper materials include microfiber, textile, and EVA for lightweight versions.

The sole consists of EVA (ethylene vinyl acetate), a flexible and lightweight material that provides shock absorption. The Milano is available in at least 11 color options, including standard colors like black, white, and brown, as well as specialty finishes in green, blue, and other hues. Price points range from $34.95 for children's EVA versions to $170.00 for premium leather with shearling lining.

The Milano is available in women's, men's, and children's sizes across two width options. The Regular/Wide width corresponds to D-EE sizing, while the Medium/Narrow width covers B-C sizing.
